New Grants Support Strategies to Shape Policy and Develop Models of Care

The Hartford Foundation Board of Trustees has approved three grants totaling $1.8 million that seek to identify potential policy improvements targeting older adults with complex and costly care needs; widely disseminate research, analysis and commentary on innovative health care models; and develop a new model of primary care for testing and evaluation that supports a patient-centered approach to the care of older adults with complex, chronic conditions. 

Each of the grants supports the Foundation’s new “downstream” strategy for grantmaking focused on putting geriatrics research and knowledge into practice to change health care delivery for older adults.

Change AGEnts Initiative Launches; Pilot Grants Linked to CoE in Geriatric Medicine Available

We are pleased to announce a funding opportunity for Hartford Centers of Excellence (CoE) Scholars in Geriatric Medicine in coordination with our new Change AGEnts Initiative. CoE Scholars in geriatric medicine can now apply for small grants of $40,000 for pilot projects and preference will be given to scholars who focus on practice change through interprofessional collaboration with other Hartford Change AGEnts.

More information can be found at ChangeAGEnts365.org and questions can be directed to the CoE National Program Office at the American Federation for Aging Research (AFAR). Stay tuned for other linked funding opportunities.

It was a fantastic, high-energy evening with Change AGEnts making connections to change practice and improve care for older adults.
