Grant Summary

The Practice Change Leaders for Aging and Health Program aims to address a crucial need for leaders in practice settings—including physicians, nurses, and social workers—with the skills and knowledge to implement services that are both cost-effective and improve the health of older adults. Under the leadership of Eric Coleman, MD, MPH, the program will provide a critically needed infusion into the delivery system of 30 health care professionals with the requisite leadership skills and content expertise to drive practice improvement. The Practice Change Leaders for Aging and Health Program leverages a legacy grant in the amount of $1.5 million from longstanding strategic partner, The Atlantic Philanthropies.
