Geriatric Interdisciplinary Teams in Practice

The five sites in the Foundation's $7.3 million Geriatric Interdisciplinary Teams in Practice initiative test models of team care for frail older people to create evidence that will help overcome barriers to teams in health care.

Four of the projects-PeaceHealth Oregon Region, Group Health Cooperative of Puget Sound, Rush University Medical Center and Intermountain Healthcare-focus on team efforts to improve the primary care of older persons. The fifth site, University of Colorado Health Sciences Center, works to improve the care process in the critical period immediately after hospital discharge.
