Since 1982, the Foundation has supported efforts to improve and integrate the "system" of services needed by older adults and to increase the effectiveness of selected components of care. The emphasis is on nationally replicable, sustainable models.

For example, grants under our "Project At Home" initiative are implementing and evaluating a novel Home Hospital model for selected older patients with conditions which would normally lead to inpatient hospital admission. The model seeks to avoid the negative consequences of both hospitalization and post-discharge complications in older adults.

Another initiative, Geriatric Interdisciplinary Teams in Practice , is a series of five grants supporting the development and evaluation of diverse approaches to using teams in clinical care of older adults, building on lessons from the Foundation's Geriatric Interdisciplinary Team Training initiative.
