This $3.8 million program was first developed in 1994 to explore alternatives to hospitalization for older adults. Too often, patients are released "cured" of their original ailment but suffer from newly acquired problems such as delirium, incontinence, immobility or the aftermath of hospital-acquired infections.
The Home Hospital intends to minimize these adverse consequences, test acceptance of the model with patients and providers, examine cost-effectiveness and ensure higher patient satisfaction and comparable clinical outcomes and safety. This grant involves four sites: Johns Hopkins University , Buffalo General Foundation, Fallon Community Health Plan and the Portland Veterans Administration Median Center.
