The Foundation supports a variety of programs to improve services available to the aging community.
The National Council on the Aging
This $1.3 million project seeks to build the capacity of community agencies to more effectively address health promotion and disease management. It also aims to improve their connections with the medical and more acute-oriented parts of the health care system.
OMEGA of Palm Beach County
The OMEGA project, with a total of $1.2 million in grants since 1999, is comprised of a broad coalition of senior services stakeholders, advances senior services in four key areas: improving community "elder readiness," increasing awareness and accessibility of resources, improving service coordination and preserving seniors' ability to live in the community for as long as possible.
Elder Network of the Capital Region
This $.5 million project assists key stakeholders in the four-county Capital District of New York State in improving the local health and supportive services available to older adults. Among its goals, the Elder Network of the Capital Region seeks to:
- create a computerized information and assistance service to collect and organize information about health and social services;
- implement geriatric health education and wellness programs at local agencies;
- create a health a faith program in which community volunteers help their peers avoid hospitalizations;
- and launch a community awareness media campaign about the needs and contributions of older people in the Capital District
