The John A. Hartford Foundation Business Innovation Award
The award was inaugurated in 2016, as part of The John A. Hartford Foundation’s grant support of the Aging and Disability Institute, to highlight the successes of community-based organizations (CBOs) that have partnered with health care entities.
The award recognizes CBOs that have a track record of developing bold, transformative initiatives designed to integrate health care and long-term services and supports. Qualified submissions will demonstrate successfully implemented and sustainable partnership models that have resulted in improved quality of life for older adults and/or people with disabilities.
The deadline for nominations is May 19, 2017.
