RFP from Hillman Foundation for New Innovations in Care Program

The Rita & Alex Hillman Foundation has launched the Hillman Innovations in Care Program, which will provide over $3 million in funding over the next five years to nursing-driven models of care that target vulnerable populations. The submission deadline for the first-round of brief proposals is May 1, 2014.

The Rita & Alex Hillman Foundation has launched the Hillman Innovations in Care Program, which will provide over $3 million in funding over the next five years to nursing-driving models of care that target vulnerable populations. The foundation has released a Request for Proposal that can be downloaded from the foundation website.

The goal of the Hillman Innovations in Care Program is to advance leading edge, nursing-driven models of care that will improve the health and health care of vulnerable populations, including the economically disadvantafed, racial and ethnic minorities, LGBQT, the homeless, rural populations and other groups that encounter barriers to accessing the healthcare services. The foundation is specifically interested in proposals that address maternal and child health, care of the older adult, and chronic illness management.

The submission deadline for the first-round of brief proposals is May 1, 2014. The foundation will notify those who are chosen to move forward in the review process by May 25th, and will require full proposals by July 10, 2014. The foundation will award two grants of up to $600,000 each in December 2014.
