Leadership for the Post-Health Reform Era: Two Foundations Fund Practice Change Leaders Program

Practice Change Leaders for Aging and Health is a national program designed to develop, support, and expand the influence of organizational leaders who are committed to achieving transformative improvements in care for older adults. Hartford grantee Eric Coleman writes in the Health Affairs GrantWatch blog about the national program that is an "investment in 'human capital' with long-term dividends aimed at preparing and positioning geriatric experts to improve care of older adults across the continuum of health care settings."

Hartford grantee Eric Coleman recently wrote in Health Affairs' GrantWatch blog about the Practice Change Leaders for Aging and Health program, which is jointly funded by the Atlantic Philanthropies and the John A. Hartford Foundation. Each foundation contributes $500,000 per year over a three-year time period. He writes:

"Our nation’s health care delivery system frequently does not meet the unique needs of older adults. Wide gaps remain between evidence-based approaches, nationally recognized best practices, and how care is currently delivered for many conditions that disproportionately affect this population. Strong leadership is needed to ensure that innovations are implemented to improve health and functional outcomes in older adults.

Practice Change Leaders for Aging and Health is a national program designed to develop, support, and expand the influence of organizational leaders who are committed to achieving transformative improvements in care for older adults. The program was conceived as an investment in 'human capital' with long-term dividends aimed at preparing and positioning geriatric experts to improve care of older adults across the continuum of health care settings. Both foundations recognized the need to equip emerging health care leaders with the unique skills and knowledge needed to transform the health care delivery system. And, since the program’s launch in 2007, mentoring has been viewed as a key element in the development of leaders to help address real-world financial, regulatory, or operations-level obstacles within a rapidly changing health care environment."
