Policy Approaches to Advancing Person-Centered Outcome Measurement
Target Audience: Policymakers, purchasers, philanthropies

This policy brief presents case study findings from nine sites and policy recommendations for developing the infrastructure necessary to deliver and measure person-centered, goal-based, integrated care to people with complex needs.

Setting Goals with Vulnerable People: A Collaborative Approach
Target Audience: Health plans, community-based organizations

Person-centered care management requires effective communication and coordination among individuals and their health care providers, as well as paid and unpaid supports. Medical providers, care managers, LTSS providers, family and friends play different and complementary roles in helping an individual meet his or her goals.
