University of Washington Social Innovation Fund Renewal 2015

Grant Summary

This grant to the University of Washington’s Advancing Integrated Mental Health Solutions (AIMS) Center will support The John A. Hartford Foundation’s federal Social Innovation Fund (SIF) initiative and develop an organizational process assessment to help spread the IMPACT/Collaborative Care model of depression treatment in primary care—a signature model of care developed with John A. Hartford Foundation support that has been shown to double the effectiveness of usual care. Funding will provide ongoing support and evaluation activities to a second cohort of Foundation subgrantees representing Federally Qualified Health Centers in the rural northwest implementing the Collaborative Care model with federal SIF funds, John A. Hartford Foundation grants, and local philanthropic matching dollars. This grant will also augment the technical assistance for team training and long-term financial sustainability for all SIF Collaborative Care sites, and improve use of their current patient registry with a new web and mobile application to enable patients to feed information directly to the care management system for ongoing, secure monitoring of their symptoms.
