The Geriatric CHAMP (Curricula for Home Care Advances in Management and Practice) Program as a Frame

Grant Summary

The John A. Hartford Foundation has awarded a grant to the Visiting Nurse Service of New York (VNSNY) to improve the health of older adults receiving home health care services by expanding the CHAMP (Curricula for Homecare Advances in Management and Practice) program. Funding will be used to support the expansion and enhancement of the CHAMP program across 600 home care agencies, establish a "community of practice" that will connect home care agencies nationally to provide assistance and foster collaboration as agencies implement geriatric best practice guidelines, and support the ongoing involvement of the expert National Advisory Council of the Hartford Foundation-supported Framework for Geriatric Home Care Excellence project. The initiative, led by the VNSNY, has the broad endorsement of key industry stakeholders such as the National Association for Home Care and Hospice, the Visiting Nurse Associations of America, the Centers for Medicare and Medicaid Services, and leading home care agencies across the country. The VNSNY is the largest and oldest home care services organization in the United States, which provided over 2.2 million home visits in 2005.
