Recently, the Health Affairs GrantWatch editor invited Eric Coleman and I to write a blog post about provisions to address transitions of care in the recently passed legislation, the Patient Protection and Affordable Care Act (Section 3026 of HR 3590). The Community-Based Care Transition Program included in the legislation is based on Dr. Coleman’s Care Transitions Intervention. The intervention uses trained nurses as Transition Coaches to provide patients and their families with the skills to manage their health as they go from the hospital to home, reducing the common causes of avoidable re-hospitalization such as medication errors.
